You will need to demonstrate good note-taking practices in accordance with organisational best practice.

Instructions:

1. Download the assignment form through this link.
   
2. Save onto your computer as IQA09_3_9_Notes + your Initials, date (yyyy/mm/dd).
   
3. You will need to upload the IQA09 form + some of the following - for example, expert witness forms, screen shots, documents, photographs, photocopies etc. to show that you have met the necessary criteria. 
   
4. Upload the evidence onto the LMS via Add submission button below.
